Allow multiple custom placeholders

- Add `customFilenamespacePlaceholders` option to allow for
multiple/custom placeholders.
- Add `dropExtensions` option to allow file extensions to be dropped
(e.g. file.test.js >> file).
- Update README to reflect new options.
- Update tests.
