metadata.py: improve handling of formats and codecs

Now we've got rid of a lot of branches and instead use four module
variables _FORMAT_MAP, _VCODEC_MAP, _ACODEC_MAP, and _SCODEC_MAP to
map between format/codec code name (emitted by FFprobe) and their
display names. Some codecs (e.g., H.264 and AAC) get special treatments
to display more details, but most don't.

This model makes the module even easier to extend: for most
formats/codecs, it's enough to add a key/value pair to one of the four
dicts. (And one or more relevant test case, to be precise.)
