test: fix the build failure caused by a new werkzeug version

* Remove the test that was checking if the X-Forward-Host is valid,
  because werkzeug abandoned this check in version 0.15.0

* Add resoning in the comment and tips on secure proxy configuration in
  docs.

* Upgrade flask-shell-ipython version requirement since the lowest
  version was not available, and caused travis builds to fail.

* Closes #43
