recommend 'pip install .' over 'python setup.py install'

ticket:231 talks about how "setup.py install" breaks because of some
problem in some other subdependency. It's probably fixable, but pip is
the modern approach. (Of course, pip bottoms out in 'setup.py install
--single-version-externally-managed', but that doesn't build
dependencies).
