Fixed an issue with filter during reconnect

Message queue wasn't locked during filter request which could cause
ordering mistakes for messages queued during a reconnect.

Added code to skip pings if a message was received in the most recent ping interval and updated the tests to make sure that works.

Added doc about very short ping intervals related to 226.

Added test program for reconnect that will force the max ping out error and
grow continuously while reconnecting. Ran test for 30M messages with > 10 reconnects
once the filter fix was in.

These cahnge caused a minor negative effect on performance.
