Addressed reviesions.

Special mentions:

  - Removed copyright comments.
  - Deleted ReadGroup and Sample.Builder
  - Revamped SampleDB
  - Add curly brackets in all single liner flow controls
  - Extracted IndexRange into is own top level class (...hellbender.utils.IndexRange).
  - Changed all NPE throws for IAE ones.
  - Refactor CountColumn managers (Cohort, ReadGroup and Sample).
     - Try to make it a bit more cohesive with ExomeReadCounts.GroupBy enum values.
     - Moved the count storing to the main class (ExomeReadCounts) to save single value array copies.
       The code doesn't look worse.

   - Quit a bit of clean up (typos, and IntelliJ warnings).
