Fix spec for date attributes and date parsing

- Permit the absence of startDate and endDate attributes; the xsd
  does not specify whether these must be present, so default to
  being optional.
- Fix parsing of dates with a precision greater than one millisecond
  (by truncating to the nearest millisecond).
- Allow dates with UTC specified or time zones.
- Permit both Sensitivity and Polynomial to be specified for a
  Response.  This is not enforced in the specification.
