Refactor difficulty calculation into one routine

This moves all of the difficulty calculation logic
into a single place which is either called from a
save handler for one Asset or in larger batches.

The difficulty score logic is now repeatable. The
formula used is always:

transcriptions * (transcribers + reviewers)

This makes it safe to run at any time. When we add
new difficulty signals such as direct user input
those values will be saved somewhere they can be
included in this calculation.
