Add support for DispatchTarget over a BitField field (#54)

* Add support for DispatchTarget over a segment of a BitField

Attribute access on a placeholder field will now
return a FieldAccessor, which behaves like a transparent
proxy over that attribute of the field.
This makes it very straightforward to specify a
DispatchTarget field which depends on just a particular
segment of a neighboring BitField.

* fields: Be more general in detecting FieldAccessor

In case we actually make subclasses in the future.

* fields: Clean up stale code and a formatting inconsistency around FieldAccessor

* fields: Update DispatchTarget docs to call out the use of a bitfield segment

* fields: Exclude FieldAccessor pack/unpack from coverage

* tests: Add tests for using the same FieldAccessor multiple times in a structure
