Fix links in README (ioam -> pyviz)

URLs in README.md weren't updated when the repository moved from ioam to pyviz. Here, we rectify that.

Some of these links still worked because GitHub automatically redirects URLs when repos move. However, some were broken. For the others, it makes sense to update them anyway to reflect the new repo

- Fixes Travis badge status (unknown -> passing).
- Fixes link to Param's website.
- Changes git clone instruction to use pyviz instead of ioam.
