Enable on demand tbl creation (#259)

* added support for enabling PAY_PER_REQUEST billing mode for SharedTable, MtDynamoDbTableDescriptionRepo, and MtAmazonDynamoDbByTable

- bug fix for tableInUse exception throwing: compares intended status as opposed to always throwing tableInUse for every non ACTIVE status
- updated SharedTableBuilder tests to work with bug fix
- added tests for AmazonDynamoDbAdminUtils

- Replaced integration test with unit tests for Pay_Per_Request testing since that is now supported by localDynamoDb testing package for MtDynamoDbTableDescriptionRepoTest and SharedTableBuilderTest

* added TableBuilder interface

* updated change log
