Consolidate block_tag_opened and xmlish_tag_opened

xmlish_tag_opened is changed to always return an array, as
block_tag_opened did, so a few use sites needed to account for that.

The blacklist changes are because xmlish_tag_opened has some smarts for
void tags that were never included in block_tag_opened, even though they
applied to some elements, like <hr>.
