Branching Phase Trajectory Example + Trajectory Phase Linking (#132)

* Adds a simple branching trajectory example.
* `Trajectory.link_phases` can now take a `connected` argument that specifies that phases are to be connected.
* For a pair of phases in a connection, the first phase can provide either initial or final values to the connection, but the second phase must be connected through its initial state/time value.
