Remove unnecessary explicit lifetimes

Most of the replacements are equivalent, and some of them relax
unnecessary constraints. For example:

    &'a mut Type<'a>

is rewritten:

    &mut Type<'_>

In that case, the lifetime of the reference need not be the same as the
one bound to the type.
