bug fix for transaction receipts with multiple transactions in one block (#268)

* bug fix

* reduce to filter

* filter logs via case-insensitive tx hash check

* Also make sure we `get` the transaction via a case-insensitive hash

* Add test to make sure things are working as they should

* Don't change the way transaction is fetched from the db at this time
