Move telemetry data into one structure (#675)

* Move telemetry data into one structure

the change is motivated by configure module change. With the change, the code can be written in data driven way.

* Rename k to telem_idx

* rename telem_idx to telem_src in telemconfig page

* reorder the fields to save some RAM

* idx from model file is 1 based

* fix a bug missing one return 1

* Leave 2 bytes as buffer for repack ld
