CA-293816: stop and rollback earlier in case of errors

In case of some IO errors, the existing code would simply ignore
them and continue. As the operation doesn't succeed as expected,
it almost always guarantees some kind of issues at later stage.
It seems a better/safer option to just error out at the point,
and let the outside exception handling logic to clean/rollback.
