Modify Citus metadata sync to be idempotent

With this change, the function to sync distribution metadata to CitusDB
can be called many times for the same table without any problems. In
addition, changes in pg_shard placement state will be updated in the
corresponding CitusDB rows (i.e. they are "upserted").
