fix bug where PHI instruction would use the wrong incoming block

the code (x || y || z) would result in a broken module for two reasons:

1) The cleanup pass was deleting blocks which are required for the
   PHI instruction to work

2) The wrong block was set for addIncoming() on the PHI instruction
   when one expression, being complex, resulted in several blocks
