Fix: don’t align error icon to label in basic form

In order to align the `X` icon (that identifies a form with errors) to
the corresponding field, the icon must be appended to the `<label>` tag
in case of basic forms, so that the `position: absolute` that Bootstrap CSS
assigns to the icon matches the desired position.

Closes #122
