Improve functions to add columns to dataframes (#125)

* Remove add_id(). It's nothing different from tidyr::unite.
* Improve examples of add_status_tree().
* Polish docs of add_subquad()
* Improve examples of add_var
* Test add_var:
    * Fix add_var to parse "NA" as `NA`.
    * Fix `start`.
* Fix add_col_row2()
* Refactor tests
* Increase cover.
* Show how to separate complex `QuadratName`.
