Fix #35 for commands that specify --account or --cluster to be handled correctly (#34)

Regardless of the current setting for default account/cluster, handle the --account/--cluster flags correctly. Adding an account or cluster will now set that account/cluster as a default as reported by nsc env. Commands that specify an --account/cluster will
use the value transiently.
