Allow assignment of arbitrary connection info

Allow assignment of arbitrary connection information through the
ConnectionOptions structure. Unexport ClientInfo renaming it to info.
Add Connection.Creation() and Connection.RemoteAddr() methods to the
Connection interface and repurpose the Connection.Info() method.
Introduce field ConnectionOptions.Info of type map[int]interface{}.
Remove the userAgent parameter in the connection handler callback.
Introduce ConnectionOptions argument to the OnClientConnected hook.
Adjust tests to the changes in the API.
