Track lifetimes in structs/views/etc properly

Basically types with embedded lifetimes are replaced by factories to allow binding lifetimes at the correct times (see #11)

This should fix a few issues:

* Handles could be bypassed by cloning the struct
* It was easy to crash the library since lifetimes were often not observed
* StructBuf was self-referencing
