plots: many bug fixes and improvements, support one-sided errors

* Don't prepend "+" to error if empty string or "+" is first character.
* If only one value, rescale so value is drawn in middle of axis.
* If no numerical values in table, draw dummy axis from 0 to 1.
* Treat a one-sided error as numerical zero for plotting (closes #122).
* When processing keys, allow for numbers now being parsed as strings.
* Strip "$" symbols from text strings rendered using d3.js (see #149).
* Add anchors to about.html and help.html via "id" attributes.
