Fully tensorize the PDF calculation (breaking API change) (#285)

* this introduces a radical change (almost rewrite) of pyhf in order to improve scaling behavior of pyhf with respect to the number of channels. Instead of computing channels one by one and concatenating the result, all channels are first concatenated and all modifications are computed on all channels simulteneously.
