feat(NewRequest,NewResponse): Return pointers to reduce memory allocation

NewRequest and NewResponse now return pointers to newly allocated
Request or Response structs. This is intended to reduce the amount of
copying and memory allocation. This requires a major version bump to v3.

BREAKING CHANGE: This changes the external facing API requiring a major
version bump to v3.
