Fix and simplify many rules

- Definition of identity has been moved from `mappings.jl` to `types.jl`.

- Outer constructors for `Adjoint`, `Inverse`, etc. throw an error for any
  argument type to prevent their direct usage.  This is needed because they
  have to exported (they serve to indicate which operation to apply) but we
  want that the user benefit from simplifications done by calls to `inv(A)`,
  `adjoint(A)` or `A'`,

- Fix construction of `InverseAdjoint` and calls like `inv(A')` or `inv(A)'`.

- Tests have been update accordingly.

- More coder tests.
