Tracking/render logic for 'is_phy_colname' - #119

Added tracking for extra meta flag 'is_phy_colname' which is set to true
for all physical columns, including when the name is shared with a
relationship. This is now used for the single-rel renderer to display
the raw, local key value for bad FKs *only* when the name is a physical
column name, and undef ('(not set)') otherwise...
