chore(Heroku): Setup and deploy Heroku pipeline (#7)

- Create pipeline
- Setup review apps (not yet activated)
- Setup automatic deployment
- Integrate Travis CI build to regulate Heroku deployments
- Create HEROKU.md file for proper documentation
- Refactor sequelize to use Database URL string
- Configure review apps environment
- Setup procfile to run migrations on successful releases
- Fix error handler not returning json
- Allow different URIs for development db and test db
- Configure Travis postgres Database
