Add support for grouping by conditional expressions

Produces the following SQL:

  SELECT CASE WHEN indicator_a between 0 and 1 THEN '0-1'
              WHEN indicator_a between 2 and 2 THEN '2' ELSE %(param_1)s
              END AS bucket,
        count(user_table."user") AS "user"
  FROM user_table GROUP BY bucket
