Add support for Rails >= 5.2 and address deprecations coming with Rails 6

Support index_name_exists? with and without the default parameter.  This maintains support of Rails 4 and Rails 5.0, and adds support for Rails >= 5.1.  The default parameter was removed in Rails 5.1.

Changed `be_success` to `be_successful` in tests to address the deprecation of `be_success` in upcoming Rails 6.
