Implement per-client concurrency limit

close #25 by implementing per-client concurrency limitation
through new connection options that must now be returned
by the BeforeUpgrade hook.
Change the interface of the BeforeUpgrade hook
to accept the result of 2 two new exported functions:
`AcceptConnection` and `RefuseConnection`.
Remove the webwire server option `MaxConcurrentHandlers`
replacing it with the new connection option.
