Standardize on `export function`-like syntax rather than a stanza of exports at the end. Ref #81.

This is less work to maintain.

Sadly, I couldn't get `export...from` to work in index.mjs; rollup invoked in fathom-trainees yielded `[!] Error: 'ruleset' is not exported by node_modules/fathom-web/index.js`.
