inclusionChecker: allow setting per-log start index in config.  (#60)

This allows the `inclusionChecker` to know a lower bound for the entries
it needs to fetch to check for certificate inclusion. This is useful
when starting `ct-woodpecker` up for the very first time to monitor
a log with many existing entries.
