COMBINED Module/Controller INTO 'ModuleController' - #74

As an inbetween state for the planned Module consolidation refactor
(#74) the RapidApp::Role::Module and RapidApp::Role::Controller have now
been replaced with a single, combined role:

  * RapidApp::Role::ModuleController

This new package is essentially a direct concatination of the original
Module and Controller packages (which now die if loaded).

This is still just temporary and will be replaced in the subsequent
stages of the Issue #74 changes...

NOTE: This change is still EXPERIEMNTAL and not yet fully tested,
although it does not appear so far that it will cause any issues based
on the known existing codebases...
