Fix checkboxes in browsers other than chrome (#122)

Firefox and IE were both showing the label to the left of the checkbox instead of to the right.

- We now use `display:inline-flex` instead of `float` to position the elements of the checkbox
- The warning glyph is now nested within the label for better vertical alignment
