Refactor indexed data packaging

Instead of just producting IndexData objects, the indexer
now produces VersionedRecord objects which themselves can
yield the IndexData objects from the record. Though this
isn't a large change it shifts the thinking slightly from
individual data objects to a group of data objects produced
from a single record.
