src: Make the MaRC input file scanner reentrant.

The MaRC input file scanner is now reentrant.  This makes it possible
to parse and scan multiple text files in parallel (relates to #33),
although this is not yet leveraged.  It also addresses reachable block
errors reported by valgrind.  Fixes #84.

MaRC input file opening and closing has also been made exception
safe.
