Route authorization (#673)

* Route level authorization for modules

* Check for authorization on module routes

* Render unauthorized component on unauthorized routes

* Rename `poc_config/clinical` to `poc_config/clinic` to be aligned with app id

* Use full screen width in `pocHeader` and `breadcrumb`

* * Use `pt` as default preferred language

* Translate unauthorized message
