Tweak mappings using separate file.

- Add separate file `mime.types.custom` for custom mappings.
- Update `bin/generate.php` to read new file with precedence.
- Update `MimeMappingGenerator` to make sure arrays are unique.
- Rebuild `mime.types.php`.
