Refactored chapter_pages to take chapter number and series
chapter_pages now takes the chapter number and SeriesCache instead of chapter url. Makes use of the cached series list to retrieve url. SeriesCache.set_chapter_list can now set the chapter list to None, for use when the index html has no index. Added more tests to cover changes.
