Improved DataFormat validation and reading of fixed rows

* Removed Cid.field_format_at(...); use the read only property Cid.field_formats[...] instead.
* Changed default line delimiter from 'none' to 'any'. In practice, this only affects fixed format data.
* Fixed reading of fixed data with mixed line delimiters.
* DataFormat.validate() can now be called only per data format instance.
* Improved error handling.
