refactor: Do not include muted topics count in subscriptions

Rework getUnreadByStream and getUnreadStreamTotal to use the same
logic and reuse the same code. Correct tests to reflect that.

Now unread counts are consistent with web app across all screens:
 * unread streams & topic tab
 * subscriptions tab
