ref #12 - work around load order issues

for the remove_comments_aggressive_flag

as ->init is called before ->minify, and we don't use $self-> in the
regexp to prevent memory leaks, we end up with load order issues - we
are checking if $remove_comments_aggressive is set *before* it will ever
be set

revealed due to the 02-io.t test failing and is fixed by recalling
->init in the case of the remove_comments_aggressive flag being set

also don't depend on the remove_comments option being set to have the
remove_comments_aggressive option work

update Changes, MANIFEST, and bump VERSION for CPAN release
