More repackaging changes (#499)

* Simple 'list-orgs.js' example of how to use udaru-core directly. Needed to add  to release the postgres client.

* Renamed packages: udaru -> @nearform/udaru-core, udaru-server -> udaru, udaru-hapi-plugin -> @nearform/udaru-hapi-plugin

* Renamed udaru-server -> udaru-hapi-server

* Minor update to example

* Minor nits in README

* Updated all readmes

* Sorted out incorrect and unused package dependencies

* WIP - moving several things around

* Moved database from udaru-test to udaru-core

* Moved bench tests from root to udaru-hapi-server

* Moved security from root to udaru-hapi-server

* Added depcheck and more package cleanup

* Moved generate coverage reports from docs to coverage directory

* Added missing uuid dependency

* Fixed swagger generation

* Moved init and policyLoader scripts

* Remove skipped tests

* Changed postgrator to migrate to 'max'

* Add testing commands to udaru-hapi-server so it can be tested from scratch
