Advanced docs. Several major bug fixes. Bump 0.0.4

* Provided examples of advanced use including filtering, excluding and limiting.
* Fixed bug in CandidateList that caused it to always show "total" counts
  in table representation even if a group filter was applied on creation.
* Fixed serious bug that prevented accurate results being gained when using
  group_buckets["total"] caused by regions being appended to the total
  group bucket before the census on other samples was complete.
* Changed behaviour of exclusions to explicity require use_chrom to enable
  chromosome specific exclusions instead of users having to provide a 0 dict.
* Fixed bug where chromosomes were not being correctly excluded when
  providing a list of names and numbers over a bool.
* Prevented "around" behaviour for upper and lower bounds of distance
  filters when using max and min sorting operations.
