Fix https://github.com/nats-io/node-nats-streaming/issues/58 (#199)

* [FIX] Handling of calling `nats.timeout()` with an SID generated by the new request/reply. This fixes https://github.com/nats-io/node-nats-streaming/issues/58.

* Changed doc on timeout to be more clear.

Removed {} from call to subscribe in createResponseMux. The IDE is showing errors but that is due to an incorrect typescript definition to be addressed in a different PR. The options are optional.
