Add rough Rails integration for migrations

- Add checkpoint:migrate Rake task
- Add Railtie to make the Rake task available to a Rails project
- Pluck ActiveRecord config out of its connection in the Railtie since
  that will probably be the default mechanism in a hook that fires
  after :load_config_initializers (if an application initializer hasn't
  set up the Checkpoint DB connection by that point)
- Set all of the permit columns to NOT NULL while we're at it
