New request reply (#195)

* New Request/Reply. Node client's `requestOne`, now makes a global subscription used for requests made by `requestOne`. A separate PR will make request use this mechanism.

- [FIX] the optional parameters in 'oldRequestOne' were not shifted correctly.

* Fix for node4/6 - Object.values() doesn't exist.

* Fixed the jsdoc, timeout parameter was elided. Also enclosed optional parameters in brackets (this is jsdoc formatting for optional).
